Overview

MassHealth is the Medicaid health insurance plan in Massachusetts. If you qualify for MassHealth, you’ll receive free or low-cost health care through the Massachusetts Health Connector.

Eligibility

There are a lot of factors that affect your eligibility for MassHealth, what you pay, who in your family is covered, and more. Income is the most important factor to determine whether you’re eligible, but you may qualify even if you have a job or private health insurance. To be eligible you must:

  • Be a resident of Massachusetts
  • Be under age 65
  • Not live in a nursing home

Parents or guardians of children under 19 and people with disabilities can also use this application to apply for MassHealth. You can apply at any time of the year.

Before you start

To apply, you need to have your:

  • Proof of residency
  • Social Security numbers for family members
  • Immigration documents for non-U.S. citizens
  • Federal tax return from last year
  • Current income (if you didn’t file taxes last year or your income has changed)
  • Mailing address for family members
  • Insurance information from your employer, if they offer it
    • Do any of their health plans meet the “minimum value” standard?
    • What is the employee contribution for the least expensive health plan they offer? How often do you pay?

How to

There are 3 steps to the application process for individuals and families.

  1. Fill out the application for MassHealth in 1 of 5 ways:

    Online

    Create an account on the Health Connector. The program will walk you through how to fill out the application. You can save your application and return to finish it later.

    In person

    Find a MassHealth enrollment center near you. You can make an appointment with someone to help you fill out the application.

    By mail

    Download the right MassHealth applications for individuals and families, depending on who you want to include on your plan. Mail them to:

    By fax

  2. Submit documents to prove you’re eligible.

    After you apply online, you’ll get a list of what documents you need to submit to the Health Connector to prove your eligibility. If you applied another way, you and everyone you listed on your application will get a list by mail.

    You have to submit all documents to the Health Connector within 90 days to complete your application.

  3. If you’re eligible, MassHealth will notify you, and you’ll be able to select one of the plans under the program.
